Does anyone know what happened to the old way to colour markers like arrowheads?

In 0.91 for MAC I can't find it in the menu... Did the procedure change?

I don't know about the Mac version, but on my Linux box I'm seeing the following:
1) Markers are now colouring to match the stroke automatically. Changing the stroke colour also changes the markers.
2) If you want a different colour, or the markers get out of sync with the stroke, there's Extensions > Modify Path > Colour Markers... which handles both colouring the markers to match the stroke and also setting a specific colour if you prefer.

As far as I know there is also a setting related to marker colouring in the preferences.

The extension is no longer needed: Inkscape 0.91 now updates (stock) marker colors to match the path's fill and stroke colors automatically (the new feature can be turned off in the preferences) - see also the brief mention in the release notes 0.91 (-> Markers).
The old extension (which was renamed to 'Color Markers') is disabled in the current stable release Inkscape 0.91 (on all platforms, not just in the packages for Mac OS X) due to to incompatibilities with the new auto-color-matching feature (apart from the fact that for default use cases (stock markers), it should no longer be required, see above).

Just curious - did your distro add it back, or are you using trunk builds? This extension was intentionally disabled in the release branch for Inkscape 0.91 ...

Neither. I'm still using 0.48 for day-to-day work, but had a 0.91 pre-release kicking around that I used to look at this. The first draft of my reply mentioned this, but I edited it out as I felt it might add confusion. Apologies for doing exactly that by not leaving it in!
